You can pay for your office visits and any treatments by cash, check, credit or debit card at the time of treatment. If you have any questions please call us at (860) 886-2497. $20.00 charge is assessed for returned checks.

For New Patients, please fully complete and sign the insurance information page we have mailed to you or you have downloaded from our site. For Existing Patients, if you have not been to our office in a while you will need to update all of your information and fill out new forms. Unless a prior arrangement has been determined, if you do not have dental insurance your payment is due in full on the date the services are rendered.




Patients with Dental Insurance
Deductibles and co-pays are due in full on the date of your services. We accept most dental insurance and will gladly process your claim. If your insurance is a PPO, HMO, or DMO, we ask you to contact your human resources at your place of employment, or your insurance company directly to find out if your Dentist is a network participant. If you have any questions we will be glad to assist you in any way we can. Please remember that dental insurance is a benefit to you and your child. If, during the course of treatment, your benefits change, the financially responsible party is accountable for all charges.
